Your Travel Adventure Begins at the Library!

If you’re making plans for international travel, you can apply for a U.S. passport at the Columbia Public Library and the Callaway County Public Library in Fulton.

These library branches offer passport services for NEW applicants. To renew your passport, please fill out and mail in form DS-82. The library does not process passport renewal applications. Passport photos can be taken at the library for an additional fee.

The passport acceptance process must be completed by the stated end times below. The process takes approximately 30 minutes per each application, so please plan accordingly.

Passport Checklist

In order to apply for a passport book or card, please read Form DS-11: Application For A U.S. Passport.

Copies of this form are available at the Library or can be downloaded from the State Department website. If you want to fill out your application form before you come in, use black ink and leave it unsigned — do not sign the application until instructed to do so during the application process.

Applicants must complete the following checklist. Missing information will result in a delay in your passport processing time.

***All persons applying for a passport must be present in person, regardless of age.***

  • Completed application
    (COMPLETE LEGIBLY IN BLACK INK. Mistakes should be crossed out with a single line with the correction written above. No white-out or scribbling.)

    • Form DS-11: Application for a U.S. Passport
      (DO NOT SIGN the application or attach your photo until instructed to do so during the application process.)
  • Payment for Applicable Fees
    Two separate fees are payable at the time of application.

    • Check (not starters/counter), cashier’s check or money order made payable to the U.S. Department of State. Families may write one check or money order to the U.S. Department of State for their passport application fees if they are applying in person at the same time. For all other applicants, separate checks must be made for each application. (See Fees section.)

    AND

    • Cash, money order or check made payable to Daniel Boone Regional Library for execution fee of $35. Credit cards also accepted at Columbia passport office.(If you need a photo taken, please add $15 to this fee.)
  • Proof of U.S. Citizenship
    (Bring one of the following original documents or a certified copy.)

    • Birth certificate issued by the city, county or state
    • Previously issued, undamaged U.S. passport**
    • Consular Report of Birth Abroad or Certification of Birth
    • Naturalization Certificate** or Certificate of Citizenship**
      **Also requires submission of certified birth certificate if applicant is age 15 or younger.
  • Most Recent Passport (if applicable)
  • Primary identification
    (Bring one of the following.)

    • Valid Missouri state driver’s license (If out-of-state, will need additional ID. See https://travel.state.gov for more information on acceptable IDs.)
    • Previously issued, undamaged U.S. passport, current foreign passport or current green card
    • Naturalization Certificate
    • Current Military ID (military or dependents) or government ID (city, state or federal)
    • For children & teens: For applicants age 16 and 17, parent/legal guardian may present primary ID if the minor applicant does not have one. For ages 15 and younger, see below.
  • Appearing parent/legal guardian (For applicants age 15 and younger.)
    • Parents/guardians must present primary identification, as listed above.
    • Applicants age 15 and younger need both parents/legal guardian(s) present to apply.
    • If only one parent/legal guardian can appear, form DS-3053 must be submitted with application.
    • If one parent/guardian has sole custody, certified proof of sole custody must be submitted.
    • Applicants age 16 & 17: It is recommended that at least one parent appear in person with the applicant to show parental awareness.
  • One Passport Photo
    • Applicant must have one official color photograph taken within the past 6 months.
    • Photograph should be 2 inches x 2 inches, full-face, front view, with a plain white or off-white background on photo paper.

    • You can have your photo taken at the library for $15 (cash, check or money order, credit or debit card).

Passport Fees

Fees for a passport book
Check or money order payable to the U.S. Department of State. No cash or credit card.

  • $130 for an individual 16 years of age or older.
  • $100 for an individual 15 years of age or younger.
  • Expedited Service Fee – additional $60 per application. This amount will be added to the processing fee in the check payable to the U.S. Department of State. No cash or credit card.
  • 1-2 Day Delivery Service Fee – additional $19.53 per application. This amount will be added to the processing fee in the check payable to the U.S. Department of State. No cash or credit card.

Check or money order payable to the U.S. Department of State. No cash or credit card.

  • $30 for an individual 16 years of age or older.
  • $15 for an individual 15 years of age or younger.

Library Fees

  • $35 processing fee per application. Money order or check made payable to the Daniel Boone Regional Library. Credit cards also accepted at Columbia passport office.
  • $15 passport photo fee. Cash, money order or check made payable to the Daniel Boone Regional Library. Credit cards also accepted at Columbia passport office.

The Columbia passport office is now accepting credit and debit cards for passport fees paid to the library. Checks or money orders are still required for the Department of State fees.
